HOWEVER, from that point Im really struggling. Im not sure how to get through the ball and release properly and end up hitting blocks or high pushes. To save that on the course I flip and fight a hook
What should I learn to get from that pre impact position with an open clubface and a bent right arm to get through the ball correctly without using my hands? Finish swivel?

Seems to me that that whilst you were mindful of a decent back swing pivot you might just be forgetting the need to pivot in the down swing and follow through also.
Your swivel, from the way you describe things, seems to be in operation but is negated somewhat by the lack of pivot through impact and on into a full finish.
